  3. Working with patients and carers • LTHT Patient Groups: • Learning Disability • Deaf and Hard of Hearing • Blind and Partially Sighted • LTHT Carers Group

  4. Staff learning • LTHT will deliver training to all grades of staff within the organisation • LTHT plan to roll out a Learning Disability awareness eLearning package

  5. Identifying patients with additional needs • The Trust Electronic Patient Administration System (PAS) has a new flag

  6. Ways of recording additional needs on PAS • Emergency admission identified as having a learning disability the A&E department • Patients admitted to ward/department who are identified as having a learning disability that is not previously recorded • In the near future patients or their carers will be able to phone to request for the flag to be added to their record NB – the above ways of recordingcan ONLY be carried out with consent

  7. Safeguarding • All LTHT staff should have level 1 (alerter) safeguarding training • LTHT has accessible safeguarding policies for adults and children which contain clear guidance for staff • Lead Nurse Safeguarding Adults • Child Protection Team (doctors and nurses)

  8. Providing information to patients and carers • New PALS leaflet • New pain assessment tool in progress • Plans to change the patient surveys & other patient information leaflets to make them easy read

  9. Reasonable Adjustments • Provide any health related information in an easy read format • Offer longer appointments so that people have the time to communicate their needs and understand treatments. • Offer the choice of first or last outpatient appointments
