Request for Call Description • The City of Phoenix is seeking a qualified consultant to provide construction administration and inspection (CM@R) services of approximately 17,000 total linear feet of 36 and 45-inch Reinforced Concrete Pipe (RCP) water transmission main to be slip lined with High Density Polyethylene (HDPE) material in Thomas Road from 44th Street to 64th Street. • One contractor will be selected for contract award.
PROJECT DESCRIPTION • As part of the Large Transmission Main Assessment Program, a need to rehabilitate a reach of transmission main along Thomas Road was identified. The project is comprised of two parallel 36-inch RCP lines between 1-ES1 (64th Street and Thomas Road) and 56th Street, and one 45-inch RCP line between 56th Street and 44th Street. A hydraulic analysis has been performed to determine maximum allowable diameter reduction. The project will install High Density Polyethylene (HDPE) slip lining material to rehabilitate this reach of the Thomas Road transmission main. • This project will be constructed under a Construction Manager at Risk (CM@R) contract.
Scope of work • Participation in final design meetings and review of construction plans and specifications • Review of and make recommendations regarding cost models/GMPs provided by the CM@R contractor and provide an independent cost estimate. • General project administration, construction inspections, and special pipeline inspections. • Provide Resident Engineering services during construction and act as the City’s representative. • Develop project schedule with the Contractor. • Provide daily interaction with contractors to clarify job requirements. • Hold on-site and off-site meetings as appropriate with Contractor, City staff, impacted third parties, utilities, and regulatory agencies as required. • Represent City and coordinate with regulatory agencies, utilities, and impacted third parties as required. • Monitoring of job progress and keep City informed of project status and issues. Provide weekly status updates. • Authorize changes as necessary due to construction or other issues arising from the work. • Issue interpretations and clarifications of the construction documents.
SCOPEOF WORK CONTINUED • Review shop drawings and test results. • Perform record keeping and maintain project documents including correspondence, schedules, submittals, test data, payments, meeting minutes, specifications, drawings, photographs, shop drawing, as-builts, etc. • Review and certification of progress payments and change orders. • Review of contractor red-lines and preparation or coordination of the development of record drawings in accordance to City standards and Water Asset Management mapping requirements. • Public information and public relation services • Coordination among City departments, governmental agencies, and operations staff • Review of and possibly create pipeline shutdown and startup plans (MOPO) • Other services as required to support successful completion of the work and the City’s interests.
Statement of Qualifications Criteria • General information (15 points) • Provide a general description of the firm or team that is proposing to provide services for the project. • Provide the following information: • List the professional licenses held by the team and the key personnel who will be assigned to this project. • Identify the location of the lead firm’s principal office and the home office location of key staff on this project. • Provide an organization chart showing key personnel. Construction Administration and Inspection Experience of the Prime Firm (25 points) • Discuss the experience and qualifications of the prime firm in providing construction administration and inspection services for similar projects. For each project listed, please provide: • Description of the project • Project’s original contracted construction cost and final construction cost • Construction dates • Project owner • Percentage of DBE participation in construction administration and inspection services • Reference information (two current names with telephone numbers per project).
Experience of Key Personnel and Subconsultants (20 points) • Experience and qualifications of the specific project team expected to be assigned to this project. Include sub-consultants’ experience. Identify each team member’s role in the projects listed. • For each key person identified, list their length of time with the firm and at least two comparable projects in which they have played a primary role. If a project selected for a key person is the same as one selected for the firm, provide just the project name and the role of the key person. For other projects provide the following: • Description of project • Role of the person • Project’s original contracted construction cost and final construction cost • Construction dates • Project Owner • Reference information (two current names with telephone numbers per project) • Project Understanding and Approach (30 points) • Understanding and approach to the project, including a discussion of the important considerations of the project and the team’s approach to the project issues. • Overall evaluation of the firm and its perceived ability to provide the required services (10 points) • Overall evaluation of the firm’s capability to provide the required services as determined by the selection panel members. No additional submittal response is required.
Submittal Requirements • Submit One (1) page cover letter • Submit the Statement of Qualifications by 12:00 noon, OCTOBER 7, 2016 • Paper Size shall be 8½”x11”; Font size may not be less than 10 point; NO MORE THAN 12 pages (including evaluation criteria and content. Does not include title page). • Pages that have project photos, charts and/or graphs will be counted towards the maximum page limit noted above • Front and back covers, cover letter, Table of Contents pages, and divider (tab) pages will NOT be counted toward the maximum page limit noted above, unless they include evaluation criteria and additional content that could be considered by the selection panel
