Telephone Triage 1300 792 977Triage Telephone Screening Assessment If the Screening Assessment indicates that an individual does not currently require specialist mental health service response, information and/ or referral to other appropriate services such as GP is provided. • Clinical Intake Assessment • Assessment conducted by appropriate team • Service response is determined • NB: Assessment is by appointment only following initial telephone screening contact with Triage Individuals assessed as not currently requiring ongoing treatment and care by the mental health service will receive information and referral options as above. Treatment and RecoveryOngoing assessment Treatment Monitoring and Review • Discharge Planning • Discharge plan includes: • Relapse Prevention Plan • Information to GP and/or other service providers • Advice on how to recontact mental health service again if necessary. • Case Closure
