  2. Work Session Think through how you will plan for your first (or next) telehealth project at your hospital/organization?

  3. What are the goal(s) of your telehealth plan? Considerations: • What issue are you trying to fix? (i.e. access to services, travel, communication, satisfaction, distance learning) • Who should you talk to about need or interest? (Clinicians, Case Managers, Nursing Staff, IT staff, Quality staff, Administration) • What is the scope of your plan? Short term vs long term • How does your plan align with organizational strategic objectives • What population(s) are you trying to serve?

  5. What solution fits your need(s)? • What of models of care are best suited for your need? • What type of personnel are involved in the workflow? • Is the work in “addition to” or “instead of existing” work • What type of technology is best suited for your need?

  6. What are the potential barriers to implementation? • Where is the funding coming from and how do you sustain it? • What are other competing projects or dependencies that may delay the plan (i.e. meaningful use)? • Who will hire/supervise the personnel resources needed for your plan? How difficult are these people to hire/find/reallocate? • If providers are involved, how will you incentivize them? • How will your plan be sustainable with evolving technologies?

  7. What will success look like? • Cost avoidance vs Revenue generation? • Do you need to track clinical outcomes? • What metrics will you use to track outcomes? How will this be done? • If successful, how will you sustain and grow the program?
