HCA’s Time Away from Work Programs Beginning April 1, 2012, we are combining your Paid Time Off (PTO), Long-Term Disabilitycoverage, Short-Term Disability benefits and Leaves of Absence. Paid Time Off Bank of hours you can use to receive your full base rate of pay during vacations or other eligible time away from work. Long-Term Disability Provides income protection if you become disabled and are unable to work for an extended period of time. Time Away from Work Leaves of Absence FMLA, state leave laws, general medical, personal, educational, military training, military active duty. Short-Term Disability Designed to replace a percentage of your pay when you are away from work due to a non-work-related personal illness or injury, based on approved medical certification.
HCA Short-Term Disability Plan & EIB • The new HCA Short-Term Disability Plan will replace the current Extended Illness Benefit (EIB) policies. • You will not lose the value of EIB hours you have earned. • Your EIB hours will form a bank of 100% Pay Replacement Hours under the new HCA Short-Term Disability Plan. • You will have access to the bank of 100% Pay Replacement Hours until you use them all or terminate employment. • The bank of 100% Pay Replacement Hours will be subject to the provisions of the HCA Short-Term Disability Plan.
Transition from EIB to Short-Term Disability • Starting April 1, you can contact the Time Away from Work Service Center for information about your 100% Pay Replacement Hours. Current EIB HCA Short-Term Disability Plan • You will no longer accrue hours in your EIB after your last payroll cycle in March (March 24 or March 31). • You will NOT lose the value of the EIB hours you have earned. They will become a bank of 100% Pay Replacement Hours in the new HCA Short-Term Disability Plan. • You will have access to the bank of 100% Pay Replacement Hours until you use them all or terminate employment. • Effective April 1, all benefits-eligible full-time and part-time employees at participating facilities will be automatically enrolled in HCA’s Short-Term Disability Plan. • You are eligible for up to 21 weeks(per event) of income replacement, including a seven-calendar-day elimination period. You can use PTO during the elimination period or take that time without pay. • Your income replacement level is based on your years of service (determined from your anniversary date).
HCA Short-Term Disability Plan: Benefits • You are eligible for up to 21 weeks(per event) of income replacement, including a seven-day-calendar day elimination period. • During the elimination period, you can use PTO or take the time off without pay. • Absences must meet the plan’s medical management criteria. • Employee continuous illness only (i.e. dependent illness not covered). • Intermittent FMLA leaves are not eligible as paid leaves under the Short-Term Disability plan. • Income replacement level is based on years of service (determined from anniversary date) and do not accrue based on pay periods:
Time Away form Work Service Center • Starting April 1, 2012, leave administration will be provided by the Time Away from Work Service Center, which is managed by Sedgwick. • The Time Away from Work Service Center will administer all new full leaves and all open intermittent leaves, including: FMLA, state, general medical, personal, educational, military training, military active duty, workers’ compensation. • The Time Away from Work Service Center does not administer leaves for bereavement, jury duty or vacation. • If you are on a full leave on April 1, 2012, the leave will continue to be administered locally and local pay practices will apply until you return to work.
Leaves of Absence: Starting April 1 • As always, you must contact your supervisor immediately to report any absence. • You must call the Time Away from Work Service Center when you will be absent from work for any illness or injury for more than two days or when you need to take a continuous or intermittent leave of absence for any reason. • 1 -855 -858-7557 • You will receive more information on the Leave of Absence program at your home address in March. The next slides will provide an overview of the Leaves of Absence process effective April 1, 2012.
The HCA Time Away from Work Programs:What You Need to Know • Paid Time Off • No action needed • You will continue to have accessto a PTO Policy. • Long-Term Disability • No action needed at this time. • If you are not already enrolled, you can enroll during your next benefit enrollment period. Time Away from Work • Leaves of Absence • A new administrative process for leaves will begin on April 1, 2012. • You will receive additional information at your home address in March. • Short-Term Disability • If eligible, you will automatically be enrolled in the HCA Short-Term Disability Plan. • If you are eligible and have less than 5 years of service, you will receive enrollment information regarding the CorePlus Supplemental Short-Term Disability Plan.
