Our mission Daybreak Independent Services Inc. is committed to develop and provide a integrated comprehensive system of services for adults with Mental retardation and wide range of Developmental Disabilities. These services are designed and developed to strengthen the abilities, improve and expand the opportunities to be independent such that adults with disabilities may live with dignity as fully functioning, participating and contributing members of our community
4. Whom will we serve?
DOE graduates diagnosed with MR/DD who desire a uniquely structured environment to foster and develop basic life and pre-employment skills
Bronx and other NY counties
The individual best suited or this program should display an interest in developing his/her basic skills along with the opportunity to eventually earn a salary
5. What services do we offer? Services
Day Habilitation

Riverdale, Bronx, NY
We expect to develop a fully operational facility within three to six months of approval (OMRDD)
Service Directions Inc, a sub work training site will serve as an employment base for our agency
The program will make use of a simulated business environment operated by the agency
6. Why are we different? We provide day program and pre-voc programs in a simulated business setting. The individuals enrolled will rotate between Day - hab and Pre-voc.
The program will take place in a community Laundromat developed and operated by DBIS
DBIS will use the Laundromat as an effective environment to put into practice the skills essential for competitive employment
The individuals enrolled will be paid for their services at the pre-vocational program starting from the sixth month of their enrolment
The program will train the individuals in specific skill sets which are formulated taking into consideration the demands of current competitive job market
The objective of the program is to get the individuals enrolled employed with in 32-36 months
7. Day habilitation
The day habilitation program will focus on developing the following skills.
These skills are identified by various job provides as essential skill
required to get a real job in today’s competitive job market.
ADL (activities of daily life) skills
ADLs are "the things we normally do in daily living, including any
daily activity we perform for self-care (such as feeding ourselves,
bathing, dressing, grooming), work, homemaking, and leisure."
ILS (Independent living skills)
Independent Living Skills are the skills needed to perform
everyday tasks (clothing, transportation, money management,
health and safety etc)
Socialisation, recreation and community integration
Team work
Basic computer proficiency
Communication skills (Verbal and Written)
Good time Keeping
8. Pre-Vocational
Our pre-vocational program will reinforce and put into practice the skills developed at the day program. In addition to that pre-voc will provide training to develop skills which are specific to real world job setting.

12. Program Model 16 individuals in day program and Pre-voc
The individuals enrolled will rotate through the pre-voc and day program with the capacity of five individuals per day
Group Day Habilitation services (GDH)
Monday to Friday 9.00 am to 3 pm
Pre-Vocational services (PV)
Monday to Friday 1:30 pm to 6:30 pm

18. OUR RESULTS AND HOW IT WILL BE ACHIEVED We expect our participant s to be employed within 32-36 months from the date of enrolment.
A five stage approach has been identified and will be used as a framework with in agency GDH and PV programs.

19. OUR RESULTS AND HOW IT WILL BE ACHEIVED
Families and individuals will be encouraged to participate at all stages of planning and development.
A program participation satisfaction survey will be implemented with each individual in the program on an annual and semi-annual basis.
The programs clinical team will also review the progress on a monthly basis and will revise the plans accordingly.
DBIS is committed to developing and defining the individuals skills to the point that they are completely employed.
DBIS is also aware that these results may not be fully attained by all individuals but makes full commitment to have all individuals reach their maximum potential.
20. Summary and Conclusion A unique agency which offer services to the MR population within the New York state with Adult Day habilitation and pre-vocational programs
Our programs will provide individuals MR/DD diagnosed with training and experience in pre-employment skill building
The project will be implemented in a real business environment utilizing resources at an agency operated Laundromat facility
The overriding objective of this project is to support individuals through a continuum of services ultimately resulting in job placement
