  1. Dermatosis Papulosa Nigra and Linear Epidermal Nevus Kirandeep Arora Team 3 Competency case # 494-1-84 Oral Diagnosis and Oral Medicine

Patient’s History • 66 year old , African American female • Pt has history of High Blood Pressure and is controlled by medication. • Pt has been blind since early childhood due to rubella infection and development of glaucoma • Multiple eye surgeries finally had her right eye removed 20 years back with no vision in left eye • Patient is a smoker

  3. Medications • Aspirin 81 mg qid • Celebrex : prn • Diovan HCT once a day • Furosemide 40 mg 2 tabs qd • Potassium Chloride 10 mg • Ranitidine 150 mg prn

  4. Reason for Dental visit • Patient seeked regular dental care • Extra oral examination revealed pigmented lesions on both right and left side of the face developed almost 10 years back • Intra oral examination revealed presence of palatal tori and bilateral mandibular tori

  5. Lesion Description • Rough, linear, well defined , brownish, pigmented area seen on the right side and middle 1/3 rd of the face. Small well defined papules seen around it • This lesion is approximately 25x9mm. • Multiple, pigmented, elevated, brownish papules seen on the left side of the face, some of which coalesce and some are scattered in the middle 1/3 rd of the face. • Lesions have not increased in size ever since they were first observed. It did increase in number.

  6. Dialogue with the Patient • When did the patient first observe these lesion? • Did any side of lesions develop first and then increased in number? • Did the size of lesion increase ever since the patient first observed? • Has the patient visited a dermatologist or oral pathologist for this? • Does any one in the family have same kind of lesions? • Does the patient have such lesions anywhere else on the body?

Diagnosis Differential Diagnosis - Left side multiple papules are diagnostic features of Dermatosis Papulosa Nigra - Right side Linear rough pigmented lesion is diagnostic feature of Linear Epidermal Nevus - Melanoma, hence pathologist and dermatologist consult is important - Congenital Nevus

  11. Provisional Diagnosis • Left sided multiple papules are diagnostic features of Dermatosis Papulosa Nigra • Right sided Linear rough pigmented lesion is diagnostic feature of Linear Epidermal Nevus

  12. Treatment • This lesion is harmless but people may undergo cryosurgery or curettage or electrocautery for the removal of these lesions for cosmetic purpose
