  2. Apr. 26 – May 02 Valparaiso April 29, 2014 Electrical panel on exit side of Conversion Press #31 was found to be open, unlocked and had low voltage wires hanging from it.

  3. Apr. 26 – May 02 Whitehouse April 29, 2014 EE alerted the Prod Control Manager that safety go/no-go dock light was damaged on one of the shipping dock door. Dock door taken out of service and lens and cover plate were replaced. Dock door was then returned to service.

  4. Apr. 26 – May 02 Winston-Salem April 30, 2014 BE Maintainer began to use platform jog control pad to jog the #3 Decorator and it started sparking. Contacted ET who found a broken ground wire. Repairs made immediately.

  5. Apr. 26 – May 02 Medical #11 in 2014 Longview May 1, 2014 Employee was loading roll of strapping onto strapper reel. Could not get it to load, core was out of round. While removing roll from reel, roll slipped out of hand. Index finger smashed between the roll and support clip for reel. Required stitches.

  6. Apr. 26 – May 02 Whitehouse May 1, 2014 During daily EHS safety walk, storage rack in parts cage found with damage to support leg. Rack had items on all 3 levels so damage compromised the rack safety. ACTION(S) TAKEN: Shelves emptied and rack area was “cautioned taped” off awaiting disassembly.

  7. Apr. 26 – May 02 Chatsworth May 2, 2014 EE rising up in a scissor lift accidentally clipped a fire sprinkler head and broke it. Fire dept isolated pipe line, line was turned off, and FD issued form for fire watch for downed affected area until repairs completed.
