What is an Incident? • In a health care setting, such as a hospital, an incident report or accident report is a form that is filled out in order to record details of an unusual event that occurs at the setting, such as an injury to a patient , accidents,or suicide
The following are some examples of reportable incidents • Error in the care of patients (e.g., errors in the administration of medications, treatments, mismatched transfusions, retained foreign bodies following surgery ).Adverse or suspected adverse reactions to procedure, medication or transfusion.
The following are some examples of reportable incidents • Development of conditions unrelated to the disease for which the patient was admitted (e.g., pressure sores).
WHEN TO PREPARE AN INCIDENT REPORT. • Reports are to be completed before the end of the shift when the incident occurred, or within 24 hours when approved by a supervisor.
What is the purposes of an Incident? • To communicate and document the occurrence of an action or situation which warrants the attention and follow-up of staff and supervisory staff. • .
What is the purposes of an Incident? • This information may be useful in the future when dealing with liability issues resulting from the incident.
WHO SHOULD PREPARE AN INCIDENT REPORT. • Any person(s) involved in or directly aware of an incident shall complete an Incident.
