Apr. 19- Apr. 25 Winston-Salem April 20, 2014 EE was walking to Sencon #2 to empty can hopper. A piece of strap laying off a pallet of scrap strap coils got caught around one of her feet and she tripped, fracturing her upper right arm.
Apr. 19- Apr. 25 Queretaro April 20, 2014 Operator was taking down a stack of 4 pallets. Bottom pallet of stack was stuck to stack behind. When pulling it out, operator lost control and two top pallets came down to the floor.
Apr. 19- Apr. 25 Bishopville April 22, 2014 Employees walking too close to maintenance work being performed on cupper. Safety chains placed in area to redirect pedestrian traffic farther away from work being performed than the already posted signs were doing.
Apr. 19- Apr. 25 Winston-Salem April 22, 2014 EE walking from Pin Oven to Printer. Stepped on rubber mat, mat slid and EE fell. Felt discomfort in knee, ice pack applied and EE returned to work. Floor had been mopped, and pad placed on floor before it was dry.
Apr. 19- Apr. 25 Whitehouse April 22, 2014 Forklift found in by battery station with forks suspended and vertical lever tied to the mast. The battery had been removed as well. Cone placed by the elevated forks until a battery was charged and the forklift could be safely operated. ITO's were consoled on how to properly notify/communicate equipment issues.
Apr. 19- Apr. 25 Fairfield April 23, 2014 EE was walking up the platform at the washer exit when they ran their hand over a conveyor support beam that was recently cut down too fit new conveying. Beam had a burr on it that could cause a cut. EE was wearing cotton gloves and did not get cut.
Apr. 19- Apr. 25 Winston-Salem April 25, 2014 EE was grinding a fitting on shop bench grinder when the fitting got caught between the tool rest and the grinding wheel. EE thumb pulled between tool rest and wheel resulting in small cut to thumb. Band-Aid applied and EE returned to work.
