BUSINESS PROPOSAL • Company Financial Information • (Section 1.15) • Respondents are advised that materials contained in proposals are subject to the Access to Public Records Act (APRA), IC 5-14-3 et seq., and, after the contract award, the entire RFP file may be viewed and copied by any member of the public, including news agencies and competitors. • Respondents claiming a statutory exception to the APRA must place all confidential documents (including the requisite number of copies) in a sealed envelope clearly marked “Confidential” and must indicate in the Transmittal Letter and on the outside of that envelope that confidential materials are included. • The Respondent must also specify which statutory exception of APRA that applies. • The State reserves the right to make determinations of confidentiality. COST PROPOSAL $460,000 is the anticipated Federal award for funding Assistive Technology Act initiatives. It is the State’s intent to use all of this funding to meet the objectives of the ATAP. Cost Proposals will be scored based on each Respondent’s ability to efficiently use the AT grant funding to maximize the benefit to constituents with Assistive Technology needs. All Cost Proposal submissions must equal the total ATA funding amount of $460,000. Cost Proposals exceeding $460,000 will be considered non-responsive and eliminated from the evaluation process. All Cost Proposals will be measured against the State’s baseline aggregate overhead rate for this project. Historically, 15% of Assistive Technology Grant Program funding has been allocated to overhead costs. Points will be awarded based on the Respondent’s proposed overhead rate below the current baseline of 15% and will be assigned as follows: To clarify, the State is interested in contracting with a vendor that both minimize overhead costs associated with the administration of the ATAP, while maximizing the objectives of the ATAP to qualified constituents. COST PROPOSAL • Each Respondent’s Cost Proposal must address the following: • The amount of the award appropriated towards overhead costs (rent, utilities, project oversight, including operational and fiscal administration, etc.) • The amount of the award appropriated towards direct service delivery (staff, equipment, etc.). This figure should be disaggregated by the other activities listed in Section 2.4 Technical Proposal: • Compliance with federal requirements, • Represent Indiana at the state and national levels, • State financing activities, • Device reutilization program, • Device loan program, • Device demonstration program, • Training program, • Technical assistance, • Transition program, and • Public awareness activities, INDIANA DEPARTMENT OF ADMINISTRATION

INDIANA ECONOMIC IMPACT The State defines FTE as a measurement of an employee's productivity on a specific project or contract. One (1) FTE equals one (1) Indiana resident employee fully engaged in the execution of activities or services germane to the scope of work included in this solicitation and the resulting contract; forty (40) hours a week, fifty-two (52) weeks a year.
