  2. Jefke, 4 jaar • Huisbezoek ‘s nachts bij Jefke (4 jaar oud) • Haalt moeilijk en zwaar adem. • Mama krijgt hem niet gekalmeerd.

  4. Anamnese • Wanneer begonnen? • Hoe begonnen: (hyper)acuut/geleidelijk? • Symptomen overdag? • Blafhoest? • Gekend met astma? Welke medicatie? • Koorts? • Corpus alienum? • Vaccinatiestatus? • Eten – slapen – spelen – pipi – stoelgang • Sufheid – prikkelbaarheid • Bekijk de context van de thuissituatie!

  5. Klinisch onderzoek • Evaluatie kroep-score • Stridor 0-2 • Retracties (intercostaal/subcostaal) 0-3 • Normale auscultatie 0-2 • Cyanose 0-5 • Bewustzijn 0-5 • Lichaamstemperatuur • Verder klinisch onderzoek op indicatie

  6. Beleid laryngitis stridulosa • Ouders geruststellen en advies geven: • Stomen niet zinvol (maar geruststellend) • Geen antibiotica!! • Afwachten • Ouders laten terugbellen om beloop te evalueren • Domus Medica • Kroepscore: 0-2  geruststellen • Kroepscore 3-7  betamethasone druppels 1.2 ml/kg • Kroepscore > 7  urgente doorverwijzing spoedgevallen • Methylprednisolone IM bij braken
