Practice Behavior 7.1 • Utilize conceptual frameworks to guide the processes of assessment, intervention and evaluation. • When a consumer comes into the agency for the first time they go through an intake process. This includes; Intakes (signing consents, emergency contact info., medical history, handbook), CCA (clinical assessment from LSCW to determine diagnosis and placement of service, (day treatment, PSR, CST) this is to be completed before seeing the doctor), Doctor appt. to discuss meds • The consumer and case manager will also discuss their skills, goals and needs. Developing a PCP to set goals to work on while their at the agency.

  4. Practice Behavior 7.2 • Critique and apply knowledge to understand person and environment. • Development of the person centered plan. • Use “” around what the consumers states and use their statements to relate back to the book and research goals and interventions to match their stated needs, goals, wants.
