Near Miss Most hazards are assigned and corrected within two days Hazard reviewed for underlying issues and risk assessment 5 workdays Monthly 24 Hours (1) Employee immediately reports any injuries to supervisor Employee (3) Supervisor works with employee to complete SIIR* investigation • (2) Supervisor ensures incident is input on Navigator and performs initial investigation (5) Supervisor submits initial SIIR* to Safety Team (Quercia/Jimenez) for review and refinement (8) Supervisor gathers all signatures and resubmits revised SIIR* • Supervisor (9) Verify corrective actions have been implemented (4) Incident is assigned (6) SIIR* received, revised and returned to supervisor Safety Team (7) Implement Corrective Actions (10) Verify corrective actions and close (11) Report is Reviewed by Incident Investigation Board * SIIR = Supervisors Incident Investigation Report Incident Investigation Board
