  4. What does LPZ measure ? 7 common problems that care home residents may face: • Pressure ulceration • Incontinence • Nutrition & hydration problems • Pain • Falls • Restraint • Deterioration

Improving care home patient safety - headlines • 450 000 care home residents • Average number of diagnoses – 6.2 • Average number of medications – 8 per resident • 30% malnourished • 56% at risk of malnourishment • Average life expectancy • 1 year for nursing homes • 2 years for residential homes

  7. How have care homes used their data to make improvements? Some examples of pressure ulcer prevention projects: • Developed link champion initiatives within homes, to upskill staff who can then cascade learning and monitor practice • Implemented regular skin inspections/body mapping as a routine • Review of cleansing/barrier products to reduce moisture associated skin damage • Ensured “React to…….” training is mandatory and firmly embedded

  8. How have care homes used their data to make improvements? Some examples of falls projects: • Introduction of safety crosses to understand at a glance the number of falls occurring • Mapping times and locations of falls and gaining a better understanding of reasons • Changing shift patterns of staff to cover times and locations most at risk • Individual resident reviews and changes to routines • Increased hydration/medication reviews
