Background Information:2015 ADSSP Grant Objective: Creating and Sustaining Dementia-Capable Service Systems for People with Dementia and their Family Caregivers • Basic Training for No-Wrong Door staff (BU Cader) • Train-the-trainer for Provider RN’s • Advanced training for ASAP Staff • Training for Informal Family Caregivers

  4. 2016 ADSSP Grant – Part B Supplementary grant to enhance 2015 Grant Activities with direct services to support family caregivers.

  5. 2016 ADSSP Grant Objectives • Increase access to Savvy Caregiver trainings by offering respite services to caregivers whose care recipients are not currently enrolled in the Home Care Program. • Promote and support the use of assistive technology for family caregivers of people living with Alzheimer’s and Related Dementia (ADRD). Special efforts should be made to reach hard to reach family caregivers such as Latinos, African Americans, LGBTQ, etc.

  6. Contracts Project Period: 8/1/2017 to 2/28/2018* Award Amount: A total of $13,375 will be distributed to the 24 ASAPs with an established Family Caregiver Support Program. Invoicing: Reimbursement Basis

  8. Contract Scope Caregivers must be enrolled in the MFCSP care enrollment prior to receiving any of these services. If appropriate they should be assessed and receive other FCSP services.

  10. SAMS Documentation Four new service categories have been added to the Massachusetts Family Caregiver Support Program (MFCSP) care enrollment in SAMS: • Respite Care ADSSP-B • Companion Person Centered ADSSP-B • Technology Counseling/Support/TA ADSSP-B • Technology ADSSP-B

SAMS Documentation Process: • Enroll caregivers in MFCSP Care Enrollment • Complete MFCSP Assessment (if applicable) • Enter all demographic data points in caregiver and care-recipient record • Service deliveries are similar to Group 1 deliveries (need to be linked to care recipient) Please note: ADSSP grant service deliveries will not be mapped to the State Reporting Tool (SRT).

  12. Direct Services Expenditures At least 50% of grant award must be spent on direct services: I. Respite Services to attend Savvy Caregiver II. Technology Supports

Direct Services: Respite Services • Only for caregivers of consumers not currently enrolled in any Home Care Program • Check care recipient record and confirm there are no Home Care service deliveries • Need respite to attend Savvy Caregiver • Special effort needs to be made to attract hard to reach caregivers

  18. II. Direct Services: Technology Supports Two types of Technology Service Categories: • Technology Counseling/Support/Technical Assistance • Specific technology awards (up to $750 per technology)

  20. Technology Counseling, Support and Technical Assistance Discovery: What are the main caregiver challenges? Problem solving: Can technology help with any of those challenges? What type of technology (from the approved list) can help? Are there apps or other no cost technologies that can be used? How do the caregivers want to use the technology? What is the path to get them to use the technology? Support and Technical Assistance: Provide technical assistance and support to ensure that caregivers are comfortable using the technology.

  25. Technology Support Provider can be contracted in SAMS GPS Tracking Device Unit is always 1 and Rate is payment amount up to $750.00

  26. LIST OF APPROVED TECHNOLOGIES

Non Direct Expenditures (up to 40%): Outreach to Hard to Reach Populations Non-direct expenditures related to planning and conducting outreach activities aimed at recruiting traditionally hard-to-reach caregivers to receive grant services (respite to attend Savvy Caregiver training and technology support) and at providing resources and information about ADRD. Please note:personnel, materials, printing, and advertising costs related to outreach activities can be included as Non-Direct Service Expenditures.

Examples of Outreach Activities • Savvy Caregiver information sessions at agencies already serving the underserved, hard-to-reach and or linguistic/cultural populations, • Disseminate Savvy Caregiver flyers in community places such as libraries, LGBTQ Elder Cafes and or social events, faith-based organizations, community health centers • Provide program flyers to Memory Café leaders including the Aroma de Café in Lawrence, so they can be included in the program resource tables.

Evaluation and Monitoring EOEA will conduct program evaluation and monitoring by: • Conducting qualitative interviews with caregivers receiving respite and or technology supports • Monitoring Service Deliveries • Auditing journal entries to get more information about caregiver challenges and types of technologies used. Please let caregivers receiving grant services know they will be receiving an evaluation call from EOEA.

  34. MFCSP invoices should NOT contain ADSSP B Service Deliveries. • Filter Agency Summary Reports by Service Categories to create separate reports.

Review carefully Business Rules, Contract Scope, and Reporting Requirements document • Decide which respite deliveries you will use and which need to be contracted in SAMS • If you have scheduled a Savvy Caregiver training, publicize the availability of respite • Decide which technology supports you will offer • Reach out if you have further questions
