How it Works: Patients complete the 5 to 10 minute screening tool in the physician’s office on a TouchScreen Monitor mounted on a stand-alone kiosk prior to their appointment. Upon completion, the system immediately generates and prints a 1-page report on a dedicated or networked computer. The 1-page report highlights elevated symptom scores, risk factors, and patient identified needs. Goal 1: To facilitate the timely screening and assessment of a variety of health and mental health difficulties that adds no additional burden to physicians’ time.
Add Features: ► Multi-lingual capacity – currently in French and English (working on Spanish and Arabic) ► Flexible – implement any screening tool quickly. ► Facilitate Self Study – monitor quality of care ► Generate Quarterly Summary Reports – monitor trends. ► Improve Knowledge Mobilization – the system can print Clinical Practice Guideline, InfoSheets, and Self-Help Guide on-demand, keyed to the patient’s / client’s identified needs. Two new grants replace the former Program Evaluation Grants: Evaluation Capacity Building Grant (ECBG) Evaluation Implementation Grant (EIG) Goals: standardization of evaluation more communities of practice established in ON Both grants will require: a readiness review if successful, regular consultation with the Centre partnerships and networks with agencies implementing similar programs and services In September 2008, the Centre funded 24 ECBGs and 13 EIGs. Next deadline is June 2009. New Funding Opportunities
Aims to: support sustainable initiatives through collaborative action promote cross-sectoral and cross-professional cooperation across the province support motivated communities in their efforts to mobilize around a pressing issue successful pilot project in 2008 has led to a full roll out of the award in 2009.
