  1. XXVIII Reunión SOGAMI. Ferrol. 2011 Leucoencefalopatía posterior reversible. Cuadro clínico inusual en paciente con síndrome Guillain-Barré y tratamiento con inmunoglobulinas iv. Leucoencefalopatia posterior reversible • Mujer de 70 años • Sind Guillain-Barré rápida evolucionTetraplejia+respirador • Administración de Inmunoglobulinas iv • Presenta mioclonías faciales, y 36-48 hs despues: coma y status epileptico • TAC: lesion hipodensa parieto-occipital bilateral en sustancia blanca con características de edema vasogénicoLeucoencefalopatia posterior reversible

  3. TAC febrero.2011 PARADA CARDIORESPIRATORIA -buena evolución -no presentó nuevas crisis-TACs y EEG de control:normalización

  4. ENCEFALOPATIA POSTERIOR REVERSIBLE Criterios definitorios: • Alteración aguda neurológica (convulsiones, encefalopatía, alteraciones visuales…) • TAC/RNM: edema vasogénico (afectando sustancia blanca predominantemente parieto-occipital) (no efecto masa) • Reversibilidad Clínica o Radiológica

  5. ENCEFALOPATIA POSTERIOR REVERSIBLECARACTERÍSTICA CLÍNICAS Series escasas, retrospectivas, limitadas Fugate et al. Mayo Clin Proc 2010: 120 casos en 10 años: • Convulsiones 74% • HTA “aguda” 86% (190/105 mmHg) • Tto inmunosupresor: 42% • Enf autoinmune 44% • Eclampsia, trasplantes,…. • Sepsis (afectación cortical)
