State of Iowa – CHNA HIP:Community Health Needs Assessment and Health Improvement Plan • State of Iowa creates a state plan that helps to address national goals • Counties in Iowa are empowered by the state to evaluate needs and set county-specific goals • The County Health Needs Assessment (CHNA) is intended to guide formation of an associated Health Improvement Plan (HIP)

  17. % of Residents with BMI over 25 Story County has a 18% increase in self-reported BMI over 25 since 2005; however, the level reported by residents is 2% lower than the national leveland 6% lower than the state level

  18. 2005 to 2010 Change in Smoking

  19. Binge Drinking (5+on one occasion) The highest percentage of binge drinking is among those in the 18-39 year-old age bracket. Males report 8% higher than women on binge drinking.

  20. Number of days of Exercise in the last week Of those who said they had exercised in the past week, only 38% exercised the recommended amount of at least 3 days.

  21. Number of fruit/vegetables servings taken yesterday 61% of respondents reported that they had 2-4 servings of fruit/fruit juice the previous day. 15% of respondents reported that they had 3+ servings of vegetables the previous day.

  25. Barriers to accessing health care in Story County (agree + strongly agree) Cost (including prescription drugs) and difficulty in getting appointments were the main barriers to health care and are significantly higher than 2005 data

  26. Diagnosis • Cholesterol continues to be the highest diagnosed medical condition

  27. High Cholesterol Since 2005, there has been a slight decrease in the number of residents being diagnosed with high cholesterol and Story County is 10% lower than the state average.

  28. Arthritis 21.9 % of all residents that live outside of Ames have been diagnosed with Arthritis.

  29. High Blood Pressure • Story County is 4% lower than the state and national levels on residents diagnosed with high blood pressure.*

  30. Traveled outside of Story County to obtain health care in the past 12 months Low-income residents are more likely to leave the county for their health care needs

  39. Access to Food Only 2.9% got hungry in the past 12 months because they can’t afford to buy food. Of those who got hungry, 85% got hungry once a week.
