3. Hours of Operation • Hours • Monday through Friday 8:00 am until 5:00 pm • Closed on Weekends and most Holidays • Other options for emergencies • Rolla Urgent Care • St John’s Walk in Clinic • Emergency Room Phelps County Regional Medical Center

Services We are an Urgent Care Clinic with follow up We take care of minor illnesses and injuries We have the ability to refer to specialists if needed We have immunizations available most of these will have a fee associated with it: Influenza, MMR, Hepatitis, Tetanus We have a part time Psychiatrist and Mental Health Nurse Practitioner We take care of Women’s Health Issues We also take care of Sexually Transmitted diseases and testing

5. Requirements • Completed Health History Form • Copy of immunization records proving 2 MMR’s (Measles-Mumps-Rubella) • We will have the immunization available for $50 for each separate shot • Completed Meningococcal wavier or proof of immunization if living on campus • Tuberculosis Testing for all International Students • Or proof of blood screening test/ Two step TB skin testing completed with in 6 months of enrollment • Anyone who has had the BCG vaccination as a child needs to have the blood screening test
