Annual Safety Survey • Log into eServices. • Your home page – My Favorites, select Safety Forms on right side and click. • On Safety Forms main page select Safety Survey on left side and click. • Select your squadron from dropdown menu. • Your Annual Safety Survey will appear. • Fill out the survey
Safety Survey Page Selected Organization-Prior Page Answer- Yes, No or N/A Provide a narrative for No and N/A Answers Provide a completion date for NO answers.
Annual Safety Survey • Answer all questions: Yes, No or N/A • All No & N/A questions should have an explanation and select a completion date. • Yes answers can also have a narrative. • Submit when complete. • Be honest, if you have not completed a task then plan to do so in the future. • Educate your members to all item on the survey: van loading, ramp/hanger safety, MSDS forms, fire extinguishers, clean storage areas, debris free walks….etc. • Survey will be approved or disapproved. • Comments will be made to correct. • Correct and re-submit. • No Survey…….No activity for your Squadron.
