Overall Change & Reconfiguration5 year restructure • Changed from modular classroom day service to a service embedded in the community • Divided service into smaller separate locations within Bandon town • Used Person Centred Planning to clearly hear and understand choices people wanted to make • Moved people who lived in congregated settings to community living
Staff needed support to adapt to new model Families – new model more challenging – Risk Rural Location – important not to ‘flood’ community based opportunities for integration Consulted, educated and received feedback throughout development -Families consulted & included all stages - Forum set up -Awareness of community sensitivities -Access other towns’ in the locality Challenges Solutions
Transport – limited ( Rural location) Management -changing role of managers -importance of networking in local community –spread over different locations Lateral thinking e.g. rural transport -Formed networking groups in Bandon town and work closely with external organisations -Importance of risk assessments and updating same Solutions Challenges
Age Profile Targeting specific community organisation & age profile All age categories Challenges Solutions
