Proforma Goal & Request • The goal of Highline Medical Group is to build a comprehensive chronic disease management program that is sustainable and that will strengthen the existing health infrastructure and help improve quality care. • Request - Create a new FT position to manage, educate, coordinate, and build a chronic disease registry with a mission to improve outcomes for our chronic disease population at Highline Medical Group.
Position Summary • Responsible for establishing, advancing and maintaining systems to manage chronic illness patients. Work closely with physicians, directors, clinic managers, and staff to integrate medical and preventative services.
Primary Duties • Set up chronic disease management database for each clinic and assist with other areas of chronic care model implementation. • Do initial data extraction. • Be trained in all details of data entry and report capabilities of the CDEMS and asthma registry systems. • Train physicians and staff in maintaining the registry.
Primary Duties (cont’d) • Be responsible for assuring that appropriate reports and documentation are maintained for all above activities. • Develop and maintain policy and procedures relative to all above activities. • Work with the grants program manager as needed to further support the chronic care models as identified.
