  2. 2014 Insurance Highlights • Medical Insurance continues with United Healthcare • Approximately 3.8 % premium increase • New Pharmacy-Optum Rx • New cards issued June 2013

  3. 2014 Medical Insurance • United Healthcare with two plan choices • Base Medical Plan: $1,500 Individual deductible • Accelerated Medical Plan: $500 individual deductible • Rx copays • Base Medical Plan: after deductible • Accelerated Medical Plan: immediate

  4. 2014 Medical Insurance • Base Plan (high deductible plan) • Available to retiree who are pre-65 Medicare eligible • After $1,500 individual deductible, Rx copays then apply • $3,000.00 deductible for family • Health Savings Account (HSA) option available • Family members collectively required to meet family deductible • Medical Reimbursement Account (MRA) option available • Two family members can meet individual deductible amount, and then family deductible reached

  5. 2014 Medical Insurance • Accelerated Plan (low deductible plan) • Available to retiree who are pre-65 and post-65 Medicare eligible • $500 Individual Deductible • $1000 for family Deductible • $10/$35/$60 Rx retail copays for the entire year, not subject to deductible

  6. 2014 Insurance • No Changes to Pharmacy Copays • Rx Tier copays at retail pharmacy • Tier 1 = $10/ Up to 31-day supply • Tier 2 = $35/ Up to 31-day supply • Tier 3 = $60/ Up to 31-day supply • Rx Tier copays through mail order • Tier 1 = $25/ Up to 90-day supply • Tier 2 = $87.50/ Up to 90-day supply • Tier 3 = $150/Up to 90-day supply

  8. Life Insurance • Retiree Life Policy through Anthem Life –no change. • $5,000 policy only for retirees who elected the policy at time of retirement • Premium due of $4.56 November 15, for 2014

  9. Medicare Eligible • Once you become Medicare Eligible notify Human Resources • Copy of Medicare Card • Complete Cross Over Form • Medicare becomes Primary • United Healthcare becomes secondary
