Turning 5 SESIS Homepage Report • SESIS will now include a Turning 5 homepage report called “Status of IEP’s for Turning 5 Students” • School and Network staff will have access to the report. • The report will include the following fields: • Student ID • Last Name, First Name • School DBN • School Name • Grade • Process Stage • IEP Notice Sent – Y/N? • Date of IEP These students will not show up on the other SESIS status reports New York City Department of Education | Special Education Student Information System Project
Turning 5 Student Data – CAP vs. SESIS • All referral processes (Initials and requests for Reevaluations) should be completed and entered in CAP. • Turning 5 student information is migrated from CAP and includes the student’s latest referral and assessment data which will populate the Special Education Summary section of the student's profile in SESIS. • SESIS will assign Turning 5 students to the appropriate location based on the location assignment available in CAP. Turning 5 – Workflow in SESIS • Central determines case assignments to schools and CSE’s. Based on central’s assignment , CSE’s then assign cases to schools and CSE’s in CAP. • The following workflow should be followed: • Create, complete and finalize the Notice of IEP • Use the Send/Print Letter link to record the outreach in the student's Event Log • Create the draft IEP • Create the Child Outcomes Summary Form • Hold IEP meeting with parent • Fax in the signed attendance page of the IEP • Fax in supporting “entry” and “exit” sections from the CSE student case file using the fax coversheet created from the template • Finalize the Child Outcomes Summary Form • Finalize the IEP New York City Department of Education | Special Education Student Information System Project
IESP and SP in SESIS • The system will now provide an IESP / SP template available from the “Create a New Document” dropdown list. New York City Department of Education | Special Education Student Information System Project
IESP and SP in SESIS • The link provided in the SESIS template will provide an MS Word version of the document to be completed. It can be completed electronically or by hand. • The completed document will be faxed into SESIS using a fax coversheet created from the template. The fax coversheet determines the IESP vs SP. • Once the completed document is faxed in, the template will be finalized in SESIS. Frequently Asked Questions Q: How do I enter the special education teacher on the Notice of the IEP? The pre-school teacher cannot be found when using the required look-up link. A: The special education teacher for the school the student WILL attend should be entered as the required participant on the Notice. The student's pre-school teacher can be added in the optional box for “other”. Q: Where will the student’s updated grade be displayed? A: The student's updated grade will be displayed on the Demographics / Contact / Enrollment section of the student profile in SESIS. Q: What do I do if a Turning 5 student is listed on the R201 CAP report? A:. All open referrals and assessments should be finished in CAP. Begin to use SESIS by creating and finalizing the Notice of IEP and IEP in SESIS.
