Grantee Responsibilities • Annual Performance Report • Report is due April 30, 2012 • ED 524B Cover Sheet is required for the Annual Report • The Cover Sheet must be signed by the authorized representative for the grant. • RSA requests the signed Cover Sheet be emailed to your RSA Project Officer. Please alert your Project Officer if you plan to fax the Cover Sheet. • REMINDER: the G5 system will be CLOSED from Wednesday April 18 to Monday, April 23, 2012.

Format for preparing the Annual performance report • Annual Performance Report includes: • 524B Cover Sheet • Executive Summary • ED 524B Form • Section A: Project Objectives Information and Related Performance Measures Data (i.e. Project Status Chart) • Section B: Budget Narrative • Section C: Additional Information (as applicable) • Note: Grantees should prepare the Executive Summary, ED 524B Form- Part A and Part B in MS Word so that they have a back up of the report.

  8. Complete and Submit a Performance Report • Click the Package Submission menu item to begin the process of completing and submitting an electronic performance report.

  9. Complete and Submit a Performance Report • The system displays your list of awards. • Click the Select option to mark the award for which you want to complete a report. • Use the scroll bar and click the Continue > button to continue to the Performance Report List tab.

Complete and Submit a Performance Report • The Performance Report Packages screen has two sections, Current Report Packages and My Performance Reports.  The Current Report Packages section lists all the packages which have not been initiated or started which are still current.  The MyPerformance Reports section lists your reports which have been initiated, submitted or which are past due.

  11. Complete and Submit a Performance Report • Click the Select option to mark the package in the Current Report Packages section that you wish to initiate. • Click the Initiate button to begin entering the report.

  12. Complete and Submit a Performance Report • The system displays a message indicating that the report package has been initiated. • Click the Vertical scrollbar to scroll down to the forms. • Note: The forms which are part of the package display with the initial status of "Draft.“

  13. Complete and Submit a Performance Report • Click the Selectoption to select the first form listed. • Click the Edit Form button to begin editing the form you selected.

  14. Complete and Submit a Performance Report • Click the Annual Performance Report option box. • This screen shows a sample of a completed first page of a report.

Complete and Submit a Performance Report • Click the Vertical scrollbar to scroll down. • This screen shows the attachment section of the report.  • Note: The title defaults automatically from the previous sections. • Click the Browse. . . button to select the attachment from your computer's directory. • Click the Form Complete option box to indicate that the form is complete, once you have selected the suitable file from the File Directory on your computer. • Click the Save & Return to Package button to save the form and return to the report package.

  16. Complete and Submit a Performance Report • The system displays a message indicating that the form was successfully saved. • Click the Vertical scrollbar to scroll down.

  17. Complete and Submit a Performance Report • You can repeat the process for each of the forms until each one is completed. • Note:You may save a draft of your work without submitting it by clicking the Save button. • Click the Preview Package button to generate a preview of the package.

  18. Complete and Submit a Performance Report • Click the Vertical scrollbar to scroll down to the package preview link which was just generated. • Click the Click here to view the Report Package Document link.

  19. Complete and Submit a Performance Report • This screen shows a sample cover sheet for a report. • Note:  You may print the report by clicking the universal print icon.

  20. Complete and Submit a Performance Report • Click the Close Tab button to close the preview of the report and return to the G5 screen.

Complete and Submit a Performance Report • The system displays a message indicating that the report was submitted successfully. • Click the Vertical scrollbar to scroll down. • Note:  If you would like to complete additional reports, you may click Done and return to your list of awards to repeat the process. • Click the Go To Home button to return to the My Home Page screen.

Edit a Performance Report Use the following directions when a section of a submitted annual report needs to be edited or updated. • Click the Package Submission menu item to begin the process of editing a performance report. • The awards associated with your user ID display. • Click the Select button to mark the award for which you would like to edit a performance report. • Click the Continue > button to continue to the list of reports which are required. • Click the Vertical scrollbar to scroll down and view the list of reports.

Edit a Performance Report • Click the Select button to mark the report you would like to edit. • Click the Edit Report button to edit the report you selected. • Click the Vertical scrollbar to scroll down to the forms. • For this example, Sections B and C of the standard ED 524B form are in Draft status.  They will be edited and completed.

  28. Edit a Performance Report • Click the Select button to mark the form you would like to edit. • Click the Edit Form button to edit the form you selected. • The form displays. • Click the Browse… button for the File field in Section B. • Select the appropriate file from the list.

Edit a Performance Report • Click the Open button to add the file to the form. • The file is now displayed in the field. • Click the Vertical scrollbar to scroll down. • Click the Form Complete option box to indicate that the form is now complete. • Click the Save & Return to Package button to save the attached file and return to the package.

Edit a Performance Report • A message displays indicating that the form was successfully saved. • Click the Vertical scrollbar to scroll down. • The status of the form has updated to "Completed." • Click the Continue > button to continue to the Summary tab. • Click the Vertical scrollbar to scroll down.

Edit a Performance Report • Click the Submit button to submit and save the edited report package. • The system displays a message indicating that the report package was submitted successfully. • Click the Vertical scrollbar to scroll down. • Note:  If you would like to edit report packages for additional awards, you may click Done to return to your awards list and repeat the process. • Click the Go To Home button to return to the My Home Page screen. • You have successfully edited a performance report.

Delete a Performance Report Use the following directions if you would like to delete a saved annual report from G5 if the wrong report was submitted in error. • Click the Package Submission menu item to begin the process of deleting a performance report. • Click the Select button to mark the award for which you would like to delete a performance report. • Click the Continue > button to continue to the list of available reports. • Click the Vertical scrollbar to scroll down.

Delete a Performance Report • Note:  You may delete a report that has the status of "Draft" or "Unsubmitted."  For this example, the report which is in Draft status will be deleted. • Click the Select button to mark the report you would like to delete. • Click the Delete button to delete the report you selected. • Click the Vertical scrollbar to scroll down on the Summary tab.

Delete a Performance Report • The system displays a message asking if you are certain that you want to delete the report package. • Click the Yes option to confirm. • Click the Submit button to submit and save your changes. • The system displays a message indicating that the report package was successfully deleted. • Click the Vertical scrollbar to scroll down. • Note:  If you would like to delete reports for additional awards, you may click Done and return to your list of awards to repeat the process.

  36. Delete a Performance Report • Click the Go To Home button to return to the My Home Page screen. • You have successfully deleted a performance report.

Unsubmit a Performance Report Use the following directions to unsubmit a performance report from G5 prior to evaluation by the project officer in order to add additional information. • Click the Package Submission menu item to begin the process of unsubmitting a performance report. • Click the Select button to mark the award for which you would like to unsubmit reports • Click the Continue > button to continue to the list of reports associated with the award you selected. • Click the Vertical scrollbar to scroll down through the list of reports.

Unsubmit a Performance Report • Click the Select button to mark the report you want to unsubmit. • Click the Unsubmit button to unsubmit the report you selected. • Click the Vertical scrollbar to scroll down and review the information on the Summary tab. • The system displays a message asking if you are certain that you want to unsubmit the report package. • Click the Yes option to confirm.

Unsubmit a Performance Report • Click the Submit button to submit and save your changes. • The system displays a message indicating that the report package was successfully unsubmitted. • Click the Vertical scrollbar to scroll down. • Note:  If you would like to unsubmit reports packages for additional awards, you may click Done and return to the list of your awards to repeat the process. • Click the Go To Home button to return to the My Home Page screen. • You have successfully unsubmitted a performance report.

  41. Scheduled System Maintenance • Beginning at 7:00 PM EDT, Wednesday, April 18th until 6:00 AM EDT on Monday, April 23rd - G5, will be UNAVAILABLE to allow for updates to the system. • System maintenance is normally performed on Wednesday evenings from 9:00 PM EDT until Thursday mornings at 6:00 AM EDT and Sunday evening from 8:00 PM EDT until Monday mornings at 6:00 AM EDT.
