  2. What is a CHIP? A CHIP is defined as: • A strategic plan to improve the health of their population. • It includes tracked prioritized health issues and measures from a broad set of stakeholders and partners. Core elements to a CHIP: • Must include participation by community partners. • Be based on data and information from the Community Health Needs Assessment (CHNA). • Identify a set of priority community health issues using community feedback.

  3. Public Health Accreditation • PPHD achieved national Public Health Accreditation in 2016 (one of only 15 in CA) • Re-accreditation commences in 2021 • Base requirements for re-accreditation include completing, in the last 5 years, the following: • CHNA • CHIP • Strategic Plan

  6. 2018 Community Health Improvement Plan (CHIP) Goal 1: Improve the health, function, and quality of life of older adults. Goal 3: Reduce chronic disease risk by creating environments and policies that support and increase healthful nutrition and physical activity. Goal 2: Improve the healthy development, health, safety, and well-being of adolescents and young adults (AYAs). Goal 4: Improve the well-being of women, infants, children and families with a focus on health equity.

  9. Goal 1: Older Adults Goal 1: Improve the health, function, and quality of life of older adults. Objective 1.1: Decrease the proportion of older adults who experience social isolation. Objective 1.2: Increase the public’s awareness of dementia/Alzheimer’s and effective strategies for reducing the mortality and morbidity of the diseases. Objective 1.3: Improve the wellness of older adults by increasing the proportion of older adults who are up to date on a core set of clinical preventive services. Objective 1.4: Improve the wellness and health outcomes of older adults in long-term care facilities.

  10. Goal 2: Adolescent Safety Goal 2: Improve the healthy development, health, safety, and well-being of adolescents and young adults (AYAs). Objective 2.1: Reduce the proportion of adolescents ages 12 to 17 years who experience severe mental health issues including major depressive episodes (MDEs). Objective 2.2: Increase perceived safety, decrease the frequency and reasons for harassment and bullying at school and public safety in the community. Objective 2.3: Reduce alcohol, illicit drugs, and other substance abuse in order to protect the health, safety, and quality of life for all, especially among youth.

  11. Goal 3: Chronic Disease Prevention Goal 3: Reduce chronic disease risk by creating environments and policies that support and increase healthful nutrition and physical activity. Objective 3.1: Reduce the rates of smoking among adults and adolescents in Pasadena Objective 3.2: Increase the proportion of people in Pasadena who have access to healthy food options. Objective 3.2: Reduce household food insecurity and in doing so reduce hunger.

  13. Goal 4: All Children Thrive (ACT) Goal 4: Improve the well-being of women, infants, children and families with a focus on health equity. Objective 4.1: Decrease morbidity from exposure to traumatic events experienced by children before the age of 18 years. Objective 4.2: Increase perinatal health among women and infants. Objective 4.3: Increase equity relating to early birth outcomes, with a focus on racial disparities. Objective 4.4: Improve the oral health of children, adolescents and mothers in Pasadena. Objective 4.5: Increase kindergarten readiness among pre-kindergarten age children in Pasadena

Human Centered Design PHAB recommended that CHIP and strategic Plan higher level of integration Kresge ELPH grant provided resources to complete “health strategist catalyst” training and strategic planning for department Human Centered Design includes community engagement, data and investments around community health improvement, using collective impact model

Gallery Walk / Strategic Plan Seven teams created human centered design solutions to CHIP community health problems Gallery Walk (during National Public Health Week) showcased work, and solicited feedback on the solutions Five-year strategic planning, integrated with CHIP objectives and projects underway Anticipated completion of strategic plan by Fall 2019
