  2. หลักการของการจ่ายค่าบริการทางการแพทย์หลักการของการจ่ายค่าบริการทางการแพทย์ • การจ่ายย้อนหลังตามต้นทุนทั้งหมด ( Reimbursement at full cost )= Fee for service (Retrospective payment system) • การจ่ายเงินตามยอดสมาชิกที่มีการขึ้นทะเบียน • (Reimbursement per capita ) • การจ่ายเงินตามการเจ็บป่วยจริงโดยมีการตกลงราคาค่าบริการล่วงหน้า • (Prospective payment system based on DRGs)

  5. DRG เป็นระบบการจัดกลุ่มผู้ป่วย แบบหนึ่ง เพื่อให้ผู้ป่วยที่มีการใช้ทรัพยากร ในการรักษาใกล้เคียงกันอยู่ในกลุ่มเดียวกัน ข้อมูลที่ใช้ในการจัดกลุ่ม คือการวินิจฉัย การรักษาผลการรักษาและข้อมูลบุคคลของผป.

  6. ข้อมูลที่ใช้ในการหา DRG Patient’s Data : PDx, SDx, Proc, Age, Sex, Weight, D/C Type DRG Grouping

  7. ปัจจุบันประเทศไทยใช้ • ICD-10TM 2007 ของไทยให้รหัสโรค • ICD-9-CM 2007 ของอเมริกาในการให้รหัสการผ่าตัดและหัตถการต่าง ๆ • 1 ตค. 53 อาจประกาศใช้ DRG Ver 5.0ใช้ ICD-10TM 2010, ICD-9CM 2010

  8. ระบบ DRG ใช้วิธีแบ่งผู้ป่วยออกเป็นกลุ่มย่อยตามกายวิภาค (Anatomical system ) โดยแบ่งเป็น 25 Major Diagnostic Category (MDC) DRG Ver5.0 เพิ่ม MDC 28 (LOS <6 hr.)

  9. ตัวแปรและคำสำคัญที่ต้องทราบตัวแปรและคำสำคัญที่ต้องทราบ • Principal Diagnosis โรคหลัก • Comorbidity โรคร่วม • Complication โรคแทรก • Procedure หัตถการและการผ่าตัด • Relative Weight(RW) น้ำหนักสัมพัทธ์ • Age อายุ • Length of Stay (LOS) จำนวนวันนอน รพ. • Discharge statusสถานภาพการจำหน่ายผู้ป่วย • Discharge type ชนิดการจำหน่ายผู้ป่วย

  11. PDx(Principal Diagnosis) - โรคหลัก ที่ได้รับการรักษา - สรุปเมื่อ discharge - แพทย์ผู้รักษาเป็นสรุป - ใช้รหัส ICD-10 - มี 1 diagnosis เท่านั้น

  12. Complication and Comorbidity (CC) • ภาวะแทรกซ้อน หรือโรคอื่นที่เป็นร่วมด้วย ทําให้มีความยุ่งยากในการรักษามากขึ้น • มีการใช้ทรัพยากรในการรักษามากขึ้น SDx => CC

  13. SDx (Secondary Diagnosis) - โรคอื่นๆ (ที่ active) เช่น โรคร่วม ภาวะแทรกซ้อน - ใช้รหัส ICD-10 - อาจมี 0 diagnosis หรือมากกว่า

  14. การผ่าตัด/หัตถการ ที่ทำในการรักษาครั้งนั้นใช้รหัส ICD-9-CMอาจมี 0 procedure หรือมากกว่า Proc (Procedures) - Operating Room Procedures - Non-operating Room Procedures

  31. MDC เป็นเลข 2 หลัก00 = Pre MDC01-25 = MDC 1 – MDC 2526 = กลุ่ม UN • DC (Disease Cluster) เป็นเลข 4 หลัก2 หลักแรกตาม MDC คือ 00-26อีก 2 หลักหลังแบ่งเป็น 2 ช่วง 01-49 ถ้ามีการผ่าตัดหรือ Procedure 50-99 ถ้าไม่มีการผ่าตัดหรือ Procedure

  32. DRG เป็นตัวเลข 5 หลัก รูปแบบ MMDDX • MM = เลข MDC (00-26) • DD = เลข DC ( Proc 01-49, NonProc 50-99) • X = PCCL (Patient Clinical Complexity Level) 0-4 • 0 = w/o CC • 1 = mild CC • 2 = moderate CC • 3 = severe CC • 4 = catastrophic CC • 9 = w or w/o CC หรือไม่ใช้ CC
