    2. Objectives Increase patient safety through the use of bar code technology for patient identification. Reduce errors in blood transfusion process using point-of-care specimen & product tracking.

    3. IPR Changes The blood product history function allows staff to review when the last type & cross was performed, what type of blood product has been dispensed/administered, the date/time and by whom. The blood product tracking function supports electronic recording of sample collection or administration of a blood product to a patient. Transfusion: Patient Identification Proxy

    4. Equipment Both anesthesia & nursing computers can be used to see blood product history Bar code scanner needed for sample collection & product administration On anesthesia computer (new computers being installed) Does not work on nursing computer

    15. Obtaining the Product Call the Blood Bank when blood is needed. Name & hospital # RBAV Blood Bank scans the product and patient bar code. Blood is sent to the OR. Nursing assistant must read the name and hospital number outloud when delivering the blood to the OR.

    23. Printing Bar Code Labels Extra labels will be printed in PEC/DOSA Can also be printed from the OR IPR application director

    26. When ID Band is Inaccessible Patient’s arm is tucked Anesthesia provider/perfusionist have been given special authorization to use a surrogate for the ID band Label on anesthesia/prefusion record Requires an additional automated check to verify that the surrogate is the same as the ID band

    27. Surrogate (Proxy) Process Prior to tucking arm: Place label on anesthesia/perfusion record “Click on Transfusions: Patient ID Proxy” Scan ID band Scan label on anesthesia/perfusion record If scans match- use record label as ID band

    31. Backups for Computers in OR HIS support 24/7 call 5-6500; after 1700 call 6-0001 Blood bar code scanning Back-up scanner in Anesthesia workroom Call HIS for assistance/replacement Ask PACU/ASC RR/DOSA to borrow a mobile scanner

    32. Key Points If you leave the bedside between scanning the patient & placing the label on the filled tube, the entire process must be redone. Always follow proper hand hygiene procedures and wipe the computer and cart down between patients. A witness is not needed for this process; the computer acts as the witness. Only anesthesia providers/perfusionists are permitted to use a surrogate for the ID band

    33. Implementation February 7 Dual process for 1 week Typenex 2 person check PLUS bar code scanning To provide additional safety & allow conversion from Typenex bands. If process proceeds as expected, two-person verification will be eliminated at that time.
