2. Objectives Increase patient safety through the use of bar code technology for patient identification.
Reduce errors in blood transfusion process using point-of-care specimen & product tracking.
3. IPR Changes The blood product history function allows staff to review when the last type & cross was performed, what type of blood product has been dispensed/administered, the date/time and by whom.
The blood product tracking function supports electronic recording of sample collection or administration of a blood product to a patient.

4. Equipment Both anesthesia & nursing computers can be used to see blood product history
Bar code scanner needed for sample collection & product administration
On anesthesia computer (new computers being installed)
Does not work on nursing computer

Requires an additional automated check to verify that the surrogate is the same as the ID band
27. Surrogate (Proxy) Process Prior to tucking arm:
Place label on anesthesia/perfusion record
“Click on Transfusions: Patient ID Proxy”
Scan ID band
Scan label on anesthesia/perfusion record
If scans match- use record label as ID band

32. Key Points If you leave the bedside between scanning the patient & placing the label on the filled tube, the entire process must be redone.
Always follow proper hand hygiene procedures and wipe the computer and cart down between patients.
A witness is not needed for this process; the computer acts as the witness.
Only anesthesia providers/perfusionists are permitted to use a surrogate for the ID band

Dual process for 1 week
Typenex 2 person check PLUS bar code scanning
To provide additional safety & allow conversion from Typenex bands.
If process proceeds as expected, two-person verification will be eliminated at that time.
