  2. TRANSFORM To make a thorough or dramatic change in the form or character of something.

  3. What is it we want to transform and why? • Better outcomes for people. • A system that helps people lead a happy purposeful life, whilst remaining as independent as possible. • Helping people to take better control of their lives and be more central to decision making.

  4. HOW? • By developing a philosophy of support that helps independence or improvement, rather than dependence, deterioration and stagnation. • By developing an integral team approach where Health and Social Care work hand in hand to meet agreed outcomes for people • This could either be Hospital or Community based.

  5. To do all this“within the constraints of a challenging financial climate”

  6. KCCA Our Involvement so far Very positive but minimal. We feel we are still at the start of the process, the understanding phase. The real hard work and decision making is still to come.

  7. How do you see yourselves and Providers in your group fitting into the process of Transformation? Providers in the Community Care Sector account for 90% of all care provided and 15% of enablement care. We see our role and vision for the future as critical to the success of this Transformation process.

  8. What is your level of support for Transformation? HIGH If it is a transformation that delivers better outcomes for people and recognises the critical work that Social Care Support Workers do as well as delivering the targeted savings LOW If it a cause for cutting costs at the expense of the vulnerable people of Kent and a devaluation of the recognition of the work done by Social Care and Support Workers

  9. Any barriers you envisage in achieving Transformation? !!LOADS!! Develop an overall vision or aim Be more productive Develop a true integrated team approach between Health – Primary Care-Social Care Trust not Task Train for Health and Social Care Workers
