Use of aspiration thrombectomy during primary PCI Results of a Canadian survey JJ Russo, V Džavík, JA Cairns, OF Bertrand, MK Natarajan, A Cheema, A Fung, SS Jolly McMaster University, Hamilton, ON Introduction Results Sixty six surveys were submitted (response rate 51%). Seventy eight percent of respondents perform >200 PCI per year. • Small randomized controlled trials suggest that aspiration thrombectomy(AT) during primary PCI improves angiographic outcomes and may reduce mortality. • Meta-analyses of AT have shown a trend towards reduced mortality but increased risk of stroke, and significant heterogeneity in outcomes associated with different techniques and devices. Aspiration thrombectomy during primary PCI Common criteria for AT use Interventionalists reporting AT complications Objectives Aspiration thrombectomy device and technique Practice patterns during PCI • Assess the frequency, criteria for utilization, technique, and complications related to the use of AT during primary PCI for STEMI. • Determine the feasibility of a large, definitive RCT of AT. Need for a Large RCT Seventy eight percent of respondents believe that a large, definite AT RCT is needed and 85% would be willing to randomize patients to such a trial. Methods Conclusion A 27 question survey was distributed to members of the Canadian Association of Interventional Cardiology (CAIC) via email and submitted anonymously online. During AT, 89% of respondents turn on suction before advancing catheter through target lesion and 87% continue to suction while removing catheter from target vessel. A large, definitive aspiration thrombectomy RCT is needed and feasible in the current era.
