HSOPS Overview • Created by the Agency for Healthcare Research and Quality (AHRQ) • Assesses patient safety issues regarding error and event reporting • Tracks changes in patient safety over time • Baseline – taken during the beginning of the project • To be administered in August 2012 for Cohort 4 • Follow-up – taken 15 months after the Baseline survey • To be administered in November 2013 for Cohort 4 • Evaluate impact of patient safety interventions

HSOPS Overview • All HSOPS communications are sent via email to the Survey Coordinator identified during unit registration • Only one Survey Coordinator is allowed per unit • A response rate greater than or equal to 60% is ideal • Data is representative and valid • Takes approximately 10-15 minutes to complete

HSOPS Overview • MHA Keystone staff to communicate directly with Survey Coordinators • Survey Coordinators communicate directly with unit staff regarding the survey • Notify unit staff of survey • Distribute survey link to unit staff and remind them to complete it • Share reports with unit staff

  6. Target Respondents • All types of staff on the unit • Staff that have worked in the unit for at least 4 weeks and approximately 20 hours per week for that unit • Physicians who admit 2 or more patients per month and spend at least 20% of their time on the unit

Timeline • Pre-Notification Email • To be sent the week of July 16, 2012 • Registration Email • To be sent the week of July 23, 2012 • Live Survey Open • August 6, 2012 through September 7, 2012 • Final Reports • To be sent by October 19, 2012 • IMPORTANT: These dates only apply to units participating in Cohort 4, beginning data collection in July 2012.

Registration Process • Confirm contact information • Enter staff numbers by job type • All types of staff who work in the unit • Refer to “Target Respondents” slide • Check “Response Status Complete” box to confirm registration • Your unit’s live survey link will be invalid until registration is complete

  17. Completing the HSOPS • To be sent August 6, 2012 • Survey Coordinator may distribute the link to the rest of the staff via email or create a shortcut on a shared computer in the unit • Registration must be complete for the survey to be accessible

  20. Response Rate Updates • Sent 2-3 times per week while the live survey is open • Target response rate is at least 60%

Use of Previous Survey Results • Results from previous surveys may be used if the following requirements are met: • Unit-level results are available • Survey was taken within the last 6 months • Multidisciplinary • Response rate of at least 60% • Survey level data (not summary data) provided in the required import file format • File format specifications available upon request

  22. Feedback Reports • State and Unit level reports will be provided 6 weeks after the close of the survey • To be sent by October 19, 2012 • Sent to the Survey Coordinator
