  4. Class Registration • You are responsible for registering for classes.   8. Benefits • Trainees are eligible for medical and dental insurance, for both themselves and their families, commencing the first day of their fellowship.  There is sometimes a charge for this coverage, depending upon the plan selected.  • A description of the programs and the medical and dental enrollment forms are available on the Benefits & Work Life website .  Forms must be turned in within 30 days of your start date.  All benefits questions should be directed to the UW Benefits Office either via email, call 206-543-2800. • Please note: As a trainee, you are not eligible for UW retirement plans, long-term disability insurance coverage or Worker's Compensation coverage.  For exceptions on being or becoming eligible to receive retirement plans please visit UW's Retirement website for more information.  In addition, the leave policies stated below supersede any UW policies.

  9. Vacation and leave • Trainees are paid for all University of Washington recognized holidays.  Link to holidays • Trainees are eligible for up to 15 days per year of sick leave with pay. • Trainees are eligible for 30 days of paid vacation per year.  • Parental Leave. Fellows may receive stipends for up to 60 calendar days. The use of parental leave requires approval by the sponsor. • A period of terminal leave is not permitted, and payment may not be made from grant funds for leave not taken. • Unpaid leave must seek approval from the NIH awarding IC for an unpaid leave of absence.

  10. WA State residency • Non-WA State residents must apply for WA State residency status 6-8 weeks before their 5th quarter in the fellowship. • Information can be found at: http://www.washington.edu/students/reg/residency/

  11. Miscellaneous & Travel Funds • Travel • Each trainee is allotted $1,000 annually for travel expenses.  • There are many rules governing travel while on University business.  Please visit the Travel Homepage for a complete list of rules and regulations.  • Miscellaneous (Books, software, supplies, registrations, dues, etc) • Each trainee is allotted $1,000 annually for purchase of items directly related to their research training or education.  These items generally include journal subscriptions, registration fees for conferences, books, software, and course/research supplies.  • You will be reimbursed for up to $1,000.

  12. Moonlighting • Internal moonlighting - In order to do internal moonlighting at UWMC, fellows must have an Acting Instructor Appointment. • External moonlighting – Professional liability coverage is not provided by the University of Washington for professional activities outside of the fellowship program.

  13. WIPS • Work in Progress Sessions are held every Monday afternoon at the Metro Park West Building, 1100 Olive Way, on the 14th floor (HSR&D Conference Room E14) from 4:00 to 5:00 PM • WIP Schedule • http://nrsafellowship.weebly.com/wips.html

  14. Information for Annual Report • On a quarterly basis, submit to Elena a list of your goals and objectives including: • Courses taken • Research projects • Publications • Grant applications • Clinical service • Conferences attended • Mentors
