The stack slid • forward and crushed the crew leader resulting • in a fracture and eventual amputation of his leg. • LEARNINGS: • Hiab Operators • Never lift a load without proper authorization and supervision. • Never lift a load unless you are trained and certified • Lifting is hazardous operation always stand clear of the dangerous area • Never operate the Hiab outside of its weight limit. • Ensure the area around a load is clear before lifting • Ensure your training is always in date.
