5. Application and Submission Information All applications must be completed and submitted in e-snaps Must include the required certifications for compliance with fair housing and civil rights requirements, program regulations, and other federal requirements 5

6. Highlights & Changes for FY2011 Questions that were previously in the Exhibit 2 application may have been changed or removed in this year�s application. Please make sure to read all questions and instructions carefully and before indicating a response. Any FY2010 data imported into the FY2011 application should be reviewed for accuracy before the application is submitted in e-snaps. 6

7. Highlights & Changes for FY2011Cont� All applicants must complete the Applicant Profile in e-snaps for FY2011 before accessing Exhibit 2. The SF-424 has now been integrated into Exhibit 2 and must be completed for all projects. The Logic Model is no longer required to be submitted with a project application. 7

8. Highlights & Changes for FY2011 Applications for renewal funding must demonstrate participation in the CoC�s HMIS, unless Federal, State, or local laws prohibit otherwise, using form 4C (Exhibit 2). All applications must include all sponsors responsible for the operation of the project. 8

9. Performance Measures All projects must include measures for: Length of time in project Recidivism Movement to and stability in permanent housing Job and income growth Applicant may indicate additional measures to chart success 9

10. FY2011 Application Submission Application Fee The Coalition for the Homeless assesses an application fee to all applicants; new and renewals The application fee for FY2011 IS 0.65% of your requested amount Example: If the grant request is for 100,000.00 the application fee is 650.00 ($100,000.00 x .0065=$650.00) 10

11. Applications Submission to CFTH Mandatory Technical Assistance is required of all renewing agencies. Please ensure sign-up before leaving conference. Submit 3 complete copies of application Ensure copies are in order Absolutely no copies will be made by staff Have application fee in hand Will not accept application without it Applications will not be accepted after the deadline date; 9/30/11 at 3:00pm. 11
