Multidisciplinaire Richtlijn Schizofrenie - 2008 • Gemiddelde gewichtstoename als bijwerking (na 10 weken): • Clozapinegebruik: 4,5 kilo • Olanzapinegebruik: 4,1 kilo • Risperidongebruik: 2,1 kilo • Tot nu toe heeft geen enkel gecontroleerd onderzoek een middel aangewezen dat deze gewichtstoename tegengaat

A 12-month follow-up study of treating overweight schizophrenic patients with aripiprazole; Schorr et al, 2008 • Non-controlled cohort studie bij schizofrenie patienten, BMI > 25 • 53 patienten • Methode: aripiprazol additie bij baseline AP (na 2-4 weken afbouw) • 29/53 patienten studie afgerond • 16 pt monotherapie aripiprazol  gem. 3.0 kg verlies • 13 pt aripiprazol met baseline AP (9 pt clozapine; 2 pt olanzapine)  gem 4.4 kg verlies Discussie: • Switch AP = hoog risico psychose terugval • Afbouw clozapine/olanzapine mede-oorzaak gewichtsverlies • Combinatietherapie vs. monotherapie: vergelijkbare gewichtsreductie

  7. Take-home message Vraag: is er evidentie voor aripiprazol additie om overgewicht als bijwerking van olanzapine gebruik tegen te gaan? Antwoord: nee; er is te weinig onderzoek gedaan
