  2. Indications • La chirurgie est le standard de traitement des adénocarcinomes du rectum • Le traitement conservateur par irradiation est une alternative : • Si CI opératoire formelle • Si refus de l’amputation

  3. Indications Traitement curatif pour tumeurs selectionnées: Tumeur Mobile <4 cm diametre bien differenciées No clinique / US perirectal

  4. T - Staging Ax. Pondération T2 Ax. Pondération T1 pT1 N0 Mx

  5. T- Staging pT2 N0 Mx

  6. Patients • 1978 - 1998 • 97 Patients (58M/39F): 69 ans • adenocarcinome primitif: • Excision Locale ou Polypectomie exclus • recidives exclues • suivi = 82 mois ( 6-208)

  7. Tumeur • Taille moyenne = 2.4 cm • Stade (Horiot) T1A = 62 T2A = 16 T1B = 7 T2B = 12 • bien differencié = 67 / autres = 30

  8. Traitement • Irradiation Endorectale • 50 kV X-Rays • 100 Gy in 4 fractions en 6 semaines • Complément curiethérapie: 68 patients • Iridium 192 • « fourchette » • 20Gy sur isodose reference (1 cm épaisseur)

  9. Résultats • Survie globale à 5 ans= 64% • Survie sans maladie à 5 ans = 71% • 2 métastases • 26 récidives locales

  10. Analyse Multifactorielle du contrôle local (Cox model) • Stade p = 0.002 RR = 4.8 • Différentiation p = 0.005 RR = 3.13 • Réponse p = 0.0002 RR = 5.14

  11. Survie • Survie sans maladie • 71 % à 5 ans 48 % à 10 ans • Survie selon réponse tumorale • Bons répondeurs: 85 % à 5 ans • Mauvais répondeurs: 23 % • Survie selon le stade • T1A 86 % à 5 ans • T2A 60 % • T1B 28 % • T2B 37 %
