  1. Factores que predicen mejor la mortalidad y la dependencia… • Alteración del nivel de conciencia al ingreso. • Edad avanzada. • Gran cantidad de sangre en la tomografía computarizada inicial.

Tratamiento clínico. • Resucitacion. • Definir: vía aérea, respiración, circulación discapacidad, exposición, entorno. • Reposo en cama: cabecera elevada a 30 g. • Escala de glasgow estricta. • Monitorizacion • Control hemodinamico: • Antes del clipaje: TAM 80-95 mmhg • Tras clipaje: TAM mayor o igual a 90 mm hg • Presencia de vasoespasmo: m o i a 110 mmhg.

  3. Tto del vasoespasmo: • Terapia triple H • HIPERTENCION • HIPERVOLEMIA • HEMODILUCION Para aumentar la PVC: 8-12 mmhg, PCAP: 8-12 mmhg. Nimodipino: 60 mg VO cada 4 hs por 21 días. Mantenerse alerta ante la aparición de hipotensión y aumento del shunt pulmonar.
