ANTHEM FOUNDATION OF OHIOORAL HEALTH CAPACITY BUILDING PROJECT Project Methods Phase I : Develop a safety net dental practice assessment tool Phase II: Implement Assessment Tool and conduct site visits with Four Oral Health Safety Net Providers Phase III: Develop Performance Improvement Plans and Funding Requests

ANTHEM FOUNDATION OF OHIOORAL HEALTH CAPACITY BUILDING PROJECT Phase 1 Prepare a practice assessment tool/pre-visit questionnaire consisting of key indicators and data elements; Pilot test the assessment tool by having each participating safety net provider fill out the questionnaire; Finalize the assessment tool based upon the responses from the practices and the steering committee

7. ANTHEM FOUNDATION OF OHIOORAL HEALTH CAPACITY BUILDING PROJECT Phase II Site visit with each participating program to gain a better understanding of each program’s unique circumstances. Interviews with key employees Facility tours and Final data collection. Prepare an assessment of each practice Evaluate and finalize the assessment tool
