  7. Dundee-Crown High School Physicals EvaluacionesMedicas • All school physicals must be submitted before school begins • Los exámenes físicos deben entregarse antes del primer día de clases

Dundee-Crown High School Tdap Vaccination Vacuna “Tdap” • All students entering ninth grade, shall show proof of receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last Dtap, DT or Td dose. • All forms must be completed and returned to the school nurse no later than August 14, 2013. Students that do not meet these requirements will not be allowed to attend school until these requirements have been met. • Todos los estudiantes entrando a noveno grado, tienen que haber recibido una dosis de vacuna “Tdap” (se define como tétanos, difteria, tos ferina de acelular) sin importar cuándo fue la última vez que se le inyectó una dosis de “Dtap”, “DT” o “Td.” • Todas las formas tienen que ser completadas y entregadas a la enfermera escolar a más tardar el 14 de agosto del 2013.
