Models to date largely unstructured • Allows for grading of importance of goals • Very little potential for aggregation • Use of more structured care plans can be personalised • Use of a more structured approach gives potential for interoperability and reporting • Builds towards the care.data requirements
At least one need, goal and activity is present in an activity bundle, this may be in a referenced sub-bundle

Plans • The October 2013 care planning content pack is the last agreed release • Related SNOMED CT subsets are being put on notice too, currently to be withdrawn April 2015 • Government and NHS England delivery target of personalised digital care planning by 2015 • Possibilities • Dependent on consultation outcome
Consultation on the proposed withdrawal of the SNOMED CT care planning content Presented by Zac Whitewood-Moores
The absence of a declared owner or sponsor for this product means that we are proposing to withdraw it and are keen to hear views and understand the impact of not maintaining this content and its associated services • We regard this as an opportunity to maximise engagement and collaborative working at and across all levels and functions • We have a number of questions to determine how Information Standards could best support the development of a standard for care planning • In addition to the formal consultation about the proposed withdrawal a discussion is also being established that can be found via www.infostandards.org/careplanning
