One response should be submitted per incident per student in this template. • If there are multiple responses to a disciplinary incident, include only the most serious response. • The exception to this is any incident that involves firearms. Each student in possession of a firearm is to be reported as a separate incident with a separate response. • The Event Identifier field is used to “link” the Student Infraction Response record to the Student Infraction record
Report student who were served in Part C and referred to Part B for Part B eligibility determination and who turn three years old during the school year • If eligible, IEP developed and implemented by their third birthday • If not eligible, was eligibility determined prior to their third birthday • Submit in the reporting period the event occurs
