Return materials to District ELP test coord. By April 1, 2011 Separate materials by document type Submit only Form B documents Place pre-id label in the “Attention: Apply label here” box Bubble in the information in the “Teacher Use Only” box School elp test administrator responsibilities
Pre-id label corrections • For inaccurate grade level: use label and bubble in correct grade on answer document • If other information is inaccurate, destroy the label and bubble in ALL of the demographic information • When replacing an answer document, shred the old one and use a new one • If no pre-id label is provided, bubble in the demographic information School elp test administrator responsibilities cont.
It is very important that the grade be correct Confirm the grade of the student Students should not be tested out of grade level! If this occurs, invalidate the student’s test

Each school receives a pre-slugged Header Sheet for each document type Verify that the school code and name are correct Place a coded School Header Sheet on top of each type of document & rubber band together Complete School Transmittal form Pack the boxes Complete one school header sheet for each document type
Return boxes to the ELP test coordinator by April 1, 2011 ELP Test Coordinators return materials to Pearson by April 4, 2011 Secure remaining Form B materials and inventory See diagram – pg 5
Count the number of boxes Check the School Transmittal Form Complete the District Transmittal Form Store or return Accommodated materials Packing boxes – see chart on page 7 Elp test coordinator instructions
Label boxes Use fill Unresolved discrepancies – contact EED Affix the blue tracking label and seal the box Elp test coordinator instructions
Pearson has provided return labels or a bill-of-lading in your Return Instruction packet FedEx, K-2 Logistics, or USPS will be used Affix the return label – make no changes Shipment procedures
Arrange for pick up in advance • Provide the following for FedEx • Phone number • Pickup address • Physical location • Estimated # of packages • YOU DO NOT NEED AN ACCOUNT NUMBER • Keep confirmation number Fedex express pickup
Call the K-2 Logistics number on Bill-of-lading • Provide the following information: • Phone number • Address • Number of packages, etc… • Pickups can be scheduled within 24 hours • Call K-2 if there are any questions or changes K-2 Logistics pickup
Use Express mail Destroy remaining FedEx or USPS labels Inventory and Secure Form B materials Track return shipment Usps express mail
The district elp test coordinator must return materials to pearson as soon as testing is completed but no later than april 4, 2011
