The Longitudinal Study of Astronaut Health (LSAH) In 1992, NASA initiated the long term investigation of morbidity and mortality associated with space flight Protocol developed based on epidemiological research principles
LSAH’s Data Problem • 357 Astronauts and 982 comparison subjects with up to 47 years of data on each • Most data is stored electronically, but in many various formats • Needed a mechanism to record and code all relevant, discrete medical events in SNOMED CT
LSAH Participant Summary • Software that implements our context-rich information model • LSAH SNOMED CT Coding Tool • Software that enables our medical coders to easily and accurately encode medical data entered in the Participant Summary
Coding Tool key design points: • Eliminate unneeded portions of SNOMED CT • Control context in the information model • Enforce strict rules for coding and post-coordination • Provide users with advanced research tools • Store codes in a hierarchical fashion
Eliminate unneeded portions of SNOMED Why? Reduce complexity Improve accuracy when coding How? Remove non-human concepts Use only portions of SNOMED that are useful to the project’s research
2. Control context in the information model Why? Reduce complexity involved in trying to capture context within SNOMED How? Design a custom context-rich information model to gather all relevant, discrete medical events for study participants
LSAH’s Information Model * * A diagnosis can be noted as being Confirmed, Possible or Ruled out
3. Enforce strict rules for coding and post-coordination Why? Improve accuracy of coding Improve accuracy of retrieval How? Use rules engine that uses a customizable rule set
4. Provide users with advanced research tools Why? Improve accuracy of coding Improve efficiency How? Provide advanced search options Provide full details for concepts Provide various hierarchical views
5. Store codes in a hierarchical fashion Why? So the concept’s original meaning as it was coded can be recreated How? Store data in attribute/value pairs along with the parent/child relationships between each pair
