  2. 3636 Staff Concerns Reporting Line This is an anonymous reporting line and is available for all staff to use. The number to call is 3636.   4. 3636 Staff Concerns Reporting Line This reporting line is not confidential, but can be anonymous. We will feedback to any callers who leave a name and contact number.

  5. What is the reporting line for? • Notifying senior members of the Trust of quality and safety concerns when other routes have failed. • We welcome comments from medical students – please would you confirm that you are a medical student when completing the form/leaving a message so that a quarterly summary of these concerns can be sent to the Leicester University Medical School for information.

  6. For UHL Staff the reporting line is NOT:- • To replace the usual managerial arrangements – if you have a concern or issue it should be discussed with your line manager. • To replace the usual (DATIX) incident reporting system. • A vehicle to air HR grievances.

  7. If you have a safety concern:- If you have concerns about a safety or a quality issue, and your concerns have not been addressed through the other reporting channels, then please call 3636/use the InSite reporting page.
