This webinar recording details reporting requirements for existing programs. Choose your program's option for 2019-2020 and report data in Excel. Document demographic makeup, cohort data, goals, outcomes, and student support initiatives. Share successes and challenges for the year and consider mentoring opportunities. Deadline for submissions is March 1, 2019.

Intended for Existing Programs 2018-2019 • Innovative Program 2018-2019 Data Report – Excel Spreadsheet • ALL PARTNERSHIPS MUST COMPLETE THE REPORT • Option 1 • Your program does not want to continue in 2019-2020. • Option 2 • Your program wants to continue in 2019-2020. • Option 3 • Your program wants to continue in 2019-2020 and wants to modify the original application

Data Reporting – Excel • Section C • #1 – First, in the row with your Option, re-iterate the CCP Requirement for which you were approved for waiver in 2018-2019 (Columns B-C/merged). • #1 – Second, in the row with your Option, identify which CCP requirement for which you are seeking waiver for 2019-2020. (Columns E-F/merged) (Note Option 1 – not continuing, box is not available.) • Example: Student Eligibility, Ohio Revised Code 3365.03 • Check your approval letter from last spring.
Data Reporting – Excel • Section C • #2 – Looking back at your application for the current year, what was your “proposed” demographic makeup of the underrepresented cohort? Describe that here. • (This will provide a complete, stand-alone document, showing what you planned to do for the year and what you actually did the for the year.)
Data Reporting – Excel • Section C • # 3– Describe the “actual” cohort for 2018-2019. Did you serve the proposed population? Include total numbers of students who met the proposed cohort makeup and those who did not. Tell us why there were students who participated if they weren’t part of the underrepresented cohort. • Reminder: Ohio Revised Code 3365.10 indicates that the Innovative Program waiver is for “programming proposed to exclusively address the needs of underrepresented student subgroups.”
Data Reporting – Excel • Section D • Share with us the goals that you identified in your program application. Provide the data collected related to those goals. • You can add new sheets at the bottom of the screen – add new tabs, but please name the tabs so that we know what you are reporting.
Data Reporting – Excel • Section E • Again, note the demographic variable • Tell us the performance outcome goal • Tell us the actual performance outcome • You can add new tabs/sheets if that works better for your goals/outcomes • See “EXAMPLES” tab

Data Reporting – Excel • Section F • Provide some kind of identifier, but nothing that can be linked to actual student name • Provide the data that you have available related to assessment. If mid-year assessment is not available, what do you have that will show us the progress of the student toward college readiness?

Data Reporting – Excel • Section J • Get the autographs! We need to know that all partners agree to the data presented within this report. We also need to know that all partners agree to the Option selected within (cease, continue, or modify). They will attest to these agreements by signing.
Data Reporting – Excel • If you selected Option 1 (in Section A), scan, save, and send this completed report to: • ccp@highered.ohio.gov • Deadline: March 1, 2019 • As Option 1, the program will not operate in 2019-2020. The waiver will no longer be in effect. • Chancellor Gardner and Superintendent DeMaria will be informed of the partners’ decision. They may choose to send a letter confirming receipt of this information.
