Phase 1 : Medication Profile Viewer Medication History MPV will give authorized health care professionals access to the complete prescription drug histories of their patients Information is based on prescription data submitted to the Drug Plan under the Enhanced Collection of Prescription Information Legislation (ADAPT) Exception Drug Status (EDS) Look-up of EDS status and coverage periods Online References

    5. Phase 2 : E-Prescribing E-Prescribing Allows authorized prescribers to directly create and print a prescription from within a patients PIP Profile. Allergies Authorized health professionals now have the ability to record and update patient allergies and intolerances. Enhanced DUR These activities allow for a detailed DUR to be completed by the PIP system at the time the prescription is created.

    6. Phase 3 : Integration Bringing PIP and user software systems together With the help of Canada Health Infoway, National standards for communication between healthcare systems have been established. PIP will be in a position to support these messaging standards in the first quarter of 2008. It will then be up to Physician and Pharmacy vendors to make these integrated services available to their users.

    7. Data Validation Health care professionals make important decisions based on information in the PIP profile. Data validation has been ongoing to ensure patient profiles are as complete as possible.

    8. Medication Reconciliation With the availability of a centrally complete patient profile we were approached by a number of RHAs in the province to see how PIP could aid in the MedRec process. In a coordinated effort we designed a MedRec form which could be accessed from anywhere in the province.

    9. Medication Reconciliation Provincial Form We will continue to work with the RHAs in the province to further refine the form. The goal is to have a MedRec form that suits the needs of the entire province. Pilot Projects Hard Evidence to validate both PIP data and the MedRec process.
