  3. RECONNAÎTRE DOULEUR MÉNISCALE • APPARITION BRUTALE • LOCALISÉE SUR MÉNISQUE • DOULEUR INTERMITTENTE BREVE FUGACE • FAUX MOUVEMENT > RÉG MARCHE • TEST MÉNISCAUX (CASSE NOISETTE) • LA NUIT

  5. TECHNIQUE APPUI IMMÉDIAT

  6. Critères pronostics • SE MÉFIER • DOULEURS DIFFUSES • CORRÉLATION DOULEUR ET EX COMPL • LÉSION MÉNISCALE IMPORTANTE • Cependant op possible si : • DEPUIS LONGTEMPS QUI S’AGGRAVE • Malgré autres ttt médicaux • mais prudence+++++

  8. TECHIQUE: REDRESSER LE TIBIA METTRE EN APPUI LA PARTIE MOINS USÉE APPUI DIFFÉRÉ

  9. RECONNAÎTRE L’ARTHROSE • DÉBUT PROGRESSIF • DOULEUR RÉG MARCHE • LOCALISÉE PT • FAIRE RX (PAS D’IRM SEULE) ARTHROSE UNILAT

  10. PRONOSTIC • TB si BONNE INDICATION • arthrose modérée, age moyen, poids raisonnable • déformation marquée • Sinon risque soulagement incomplet et prothèse plus difficile après

  11. Prothèses unicompartimentales TB mais Indications limitées: Poids, Pas d’atteinte pivot central Pas de déformation importante Usure localisée (pb maladie inflammatoire)

  12. PROTHÈSES TOTALES INDICATIONS • Usure globale ou importante • Pas d’autre solution

  13. Appui complet immédiat cannes (4-6 sem) • H 8 jours +/- centre pronostic • EXCELLENT CHEZ SJT AGÉ PEU ACTIF • Marche, vélo, piscine
