This report highlights safety incidents, performance trends, and improvement opportunities in a mine, stressing prevention and continuous training. Positive development and operational data are included.

Safety Performance With an understanding that all incidents are preventable, and all tasks can be completed without harm, extra focus, training and improved supervision must occur to reduce the Frequency of incidents. Jun Safety performance was on acceptable level without any recorded LTI. First Aid case reported when a workman was removing a shank from simba 4 , during hammering a small piece of iron form shank hit to left arm caused small cut injury in the same. This was later found to still be in the arm and was removed a week later. First Aid case reported when a workman was opening the vent door he sustained a cut on his right hand ring finger . First Aid case reported when a workman was picking up the fire extinguisher from Volvo 3 work basket, the fire extinguisher lever was pressed and dry chemical powder was squirted from the cylinder and went into the operators face and his left eye. He was treated at the Zinc hospital. First Aid case reported when a workman was De-Rigging Solo2 to move to a new drill site (ND-105 127 XC ). Whilst stacking drill rods against the sidewall a drill rod moved unexpectedly causing a crushing to his left hand against another rod. There have been multiple dehydration issues with personnel through the month due to extreme conditions underground.
PropertyDamage Frequency of equipment damage and near incidents across Jun was higher than expected and we see this is an area that needs improvement across the board.
Safety Positives • Action Plan • Continuing delivery of SOPs to all employees.( role specific SOP completion has been targeted to ensure coverage of specific requirements) • To complete SOP coverage for all new employees before release into the work teams. • Safety Tools box talks and campaigns on:- • Heat Exposure and associated illnesses (Operational exposures) • Contact with electrical sources (Safety Theme) • Unauthorized equipment operation. Pre-start inspections. • Incident reporting. Accessing active levels. • 4. Safety tool box talks are being delivered to all employees at start of every each shift. • Reviewing, briefing and enforcement of all SOPs. • Ensuring that supervision is adequate for all employees performing tasks. • Continued operator authorizations for new equipment coming to site.
Performance Review Jun was an another positive month for development. • Overall mine performance for Jun month was 1255.11 meters. • AAC performance 696.09 meters. • RDC performance 559.02 meters. Production Drilling • Production Drilling for this month was 9663.2 meters.

SOLO Performance • Total All In Meters for June = 9043.5 mtrs • Availability vs. Utilization combined was 58% vs. 44% (Excessive back reaming, bogged Rods as Percussion not used) • Total combined Power Pack Hours = 343.9hrs • Total combined Percussion Hours = 140.8hrs • Drilling Utilization = 71.5% • Completion Rate from Designed Meters was 95.6% • Combined Percussion Rates for Month at 63Mtr/per/hr • 700mtrs achieved in 22hrs of Drilling time. • Drilling areas becoming more readily available for Month of Jun

Graph Indicators • This month AAC has achieved 92.92% of our target. • Shotcrete machine breakdowns and idle times are high and require additional management focus to increase total Shotcrete placement.-Use of Shotcrete bore holes to be optimised • This month hauling higher then pervious month tonnes. • Production Drilling was below from target. • Long round “pull length” slightly below than compare to pervious month, however the overall results must improve to achieve the maximum “pull length” in each and every round blasted. • Additional focus required to expedite consistent development in Block 1.
Improvement Opportunities and Special Focus Areas • To increase the “pull length“ on all rounds (AAC are currently reviewing processes to identify improvement opportunities • Optimising the use of B1 and B2 for longer drill rounds and replacing one boom by split feed boom to utilized in scaling, rock bolting etc.). • Additional works on improving the secondary ventilation (standards and management of vent in headings) • Training of AAC Supervisors re standards- efficiencies and correct cycling. • Standards to be driven to ensure that all “last rounds” are supported without delay. • Cycling of P1 headings and ensuring that they are not missed (maintain focus on daily targets but not at the expense of sacrificing critical headings).
SS-1 Winding Temperature Trip Provision • Kaizen: Projection Report • Problem Statement: Presently Substation-1 transformer is not having winding temperature protection. • Impact of the problem: In case of high loading or any winding fault the transformer may get damaged on account of high winding temperature. • Objective of the project: Taking the winding temperature tripping in line • Root Cause/s: No temperature feedback was taken from transformer protection relay to HT side Transformer feeder • List of solution/s identified: • Cable connection from HT TR OG breaker to transformer protection relay. • Configuration of auxiliary relay-1 for alarm & auxiliary relay-2 for tripping • Review of trip set points & relay observation. • Implementation plan and compliance:

Improvements • Solo-2 has had a number of parts removed to keep Solo1 going due to no Critical Spares on-site. • Additional experienced personnel for the Solo drills have been sourced to support the process. • Continued focus on improving the utilization of equipment. • Chatter Singh has been undertaking multiple training sessions on various equipment's and gaining competency authorisations. • AAC is currently identifying operational training improvement opportunities to support departmental requirements. • Communications between HEMM, OEM’s and AAC regarding maintenance schedule times has improved. • PDI personal’s training started on different equipments. • Scope of work and proposal being compiled for amalgamation of HZL and AAC safety and Training Departments.
SOP & Equipment Training Training for the month of Jun 2016 was as follows: SOP Trainings:- Forty one SOP’s training was given to personnel, Coupling and uncoupling of trailing cables, PPE, Drilling and Blasting etc. Mobile Equipment training & Authorization:- Loader, Explosive Carrier, PC, Scissor Lift, Volvo, LMV, Mine Truck, Miller.
SOP Training • SOP training:- This included different types of SOP training i.e. Sop Use of Self Rescuer , Coupling and uncoupling of trailing cables, Job safety environmental analysis, Drilling and Blasting etc.
Mobile Equipment Training Mobile Equipment training & Authorization :-Total 30 employees were trained and authorized on different mobile equipment: i.e. :- Cat AD 60, Loader, Boomer, Jumbo, PC, Volvo, Mine Truck, LMV.
