COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO ACUERDO DIFERENCIAS • Frecuente • Poliarticular • Simetrico • Precede o coincide con síntomas sicca • Compromiso leve • No erosiva • Compromiso axial • Asociación con auto Ac • Asociación con otras manifestaciones SÍNDROME DE SJÖGREN PRIMARIO + POLIARTRITIS VS SÍNDROME DE SJÖGREN SECUNDARIO A ARTRITIS REUMATOIDEA

COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO OBJETIVOS: Describir la frecuencia de artritis y/o artralgias en pacientes con SSj primario Evaluar las características clínico, radiológicas y ultrasonográficas del compromiso articular Analizar la asociación del compromiso articular con otras características clínicas y serológicas

COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO CRITERIOS DE INCLUSIÓN • Pacientes con diagnóstico de SSj Primario según criterios Americano-Europeos 2002 • Mayores de 18 años • Ambos sexos • En seguimiento activo ingresados en la base de datos del GESAR Sjögren a partir de Enero de 2011 con actualización anual

  6. COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO CRITERIOS DE EXCLUSIÓN • Pérdida de seguimiento en los últimos 6 meses • Fibromialgia • Dolor de origen no articular (tendinitis, traumatismos, etc)

  7. COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO TIPO DE ESTUDIO • Multicéntrico, observacional, longitudinal, analítico.

  8. COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO VARIABLES A CONSIGNAR: • Artralgias = dolor articular sin inflamación • Artritis = inflamación y/o derrame articular • Patrón del compromiso articular: - Mono, Oligo o Poliarticular - Articulaciones afectadas - Intermitente, Persistente • Rigidez matinal • Evaluación funcional: HAQ

COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO VARIABLES A CONSIGNAR (continuación): • Radiografía: erosiones • Ultrasonografía: derrame art, hipertrofia sinovial, Power Doppler, erosiones • Laboratorio: - Factor Reumatoideo - APCA (Anti CCP o Anti Vimentina) • Tratamiento • Respuesta al tratamiento (incluye eventos adversos)

  10. COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIO VARIABLES A CONSIGNAR (continuación): • Criterios de Clasificación para AR: - ACR 1987 - ACR/EULAR 2010 - Diagnóstico según especialista • Criterios de Clasificación para otra ETC

  12. COMPROMISO ARTICULAR EN PACIENTES CON SÍNDROME DE SJÖGREN PRIMARIOSUGERENCIAS • Realizarlo en forma retro-prospectiva • En las evaluaciones agregar DAS28 • Los que tienen posibilidad de realizar RM en lugar de ecografía articular consignarán los hallazgos de dicho estudio
